Self-Determination in Long Term Care: Advanced continues the discussion begun in the Basics course and teaches how Person-Centered Planning can be useful in helping people explore their options in leading self-determined lives. This course should take approximately one and one-half hours to complete. This would be appropriate for NBCC, QIDP.
Person-Centered Planning and Self-Determination
The final course in this section revolves around person-centered planning and how this framework stands as the true building blocks for self-determination. The course expert provides an in-depth conversation and explanation of Person-Centered Planning while providing additional best practices for implementing it with end clients.
Support Services and Community Living
This course also provides a deeper dive into Community Living and group support services that are available and fit into the person-centered planning framework. The services include group living and out of home support that may need to be implemented as part of the clients' treatment plans.

Elizabeth Gallagher
Program Manager
Ms. Gallagher has worked for the Michigan Department of Community Health (MDCH) since 2000. She is a Program Manager for the MI Choice waiver. Ms. Gallagher oversees the monitoring clinical monitoring and develops operational criteria, program initiatives and improvements. Prior to MDCH, she worked for the Tri-County Office on Aging for as an Eligibility Specialist and with the Medicare Medicaid Assistance Program. She holds a Master of Public Administration degree from Western Michigan University.
Tari Muniz
Ms. Muniz was hired in April 2005 by the Michigan Department of Community Health to coordinate the development and implementation of the Self Determination in Long-Term Care option of the MI Choice waiver. She works statewide to support this program. She also coordinates efforts to incorporate Person-Centered Planning in the long-term care array of supports and services. Ms. Muniz has a Bachelor of Arts degree in English from Michigan State University.
