Definition:

A Relapse Prevention Plan is a person-centered tool that identifies likely relapse risks and triggers, and sets a structured plan of action for coping with these risks and triggers if or when they may occur. Relapse prevention is a Cognitive-Behavioral Therapy (CBT) tool and has been found to be extremely effective.
